How do I know if I qualify for home care services?

 In order to qualify for the CCC Plus Waiver, an individual needs to meet the criteria in the   following areas:

• Financial Limits

• Functional Needs

• Medical Nursing Needs

• Risk of Nursing Facility Placement

How do I get started? 

The first step would be to contact us, we would then provide you with your local health department's adult and aging division in order to set up an appointment for the in-person assessment.

How long does the process take?

Depending on the county you are located in, it could take anywhere between 30-45 business days for approval from your local health department.

What happens once I am approved?

Once you are approved you would ask your local health department to provide us with the UAI, this can be faxed or emailed. We would begin services within 24-48 hours upon receival.
